FINAL FORMS
Beginning July 1, 2024, all student-athletes must register with Final Forms. Once the Final Forms registration portal opens for an interscholastic sport season, log into Final Forms to enroll your child in their desired sport. You will then be prompted to complete the required information (ie. parent signature, student signature, update form). All of this information, in addition to your valid school health examination, must be completed in order for the school doctor to be able to review/clear your child for tryouts.
